Get Waivered in 3 Steps

Step 1

Complete the free 8 hour online course or in person course and the additional 16 hour specialized training and your post course survey - then proceed to Step 2!

Step 1: Take Online Course – Part 1 – 8 hour

Step 1: Take Online Course – Part 2 – 16 hour

Step 2

Once you have finished the course and survey you will receive your Certificate of completion and you can then submit your Notice of Intent Form.

Before starting this application, please make sure you have:

Your DEA Number
Your State Medical License Number
Your Training Certificate Information

After you’ve completed Steps 1-2 above, send your certificate of completion for the 8 hour training to SAMHSA.

Email these to

infobuprenorphine@samhsa.hhs.gov.

Within 45 days you will receive your DEA X waiver.
